Been on phones for hours – how the heck do I find the cost of bloodwork!

Hey y’all – so I’m currently in the U.S. on a visitor’s visa and am planning to head back home in 4 months. I have routine bloodwork to do that I have put off for some time, but I am off my employer’s insurance (didn’t opt for COBRA, probably foolish) and signed up for this 1Enrollment PHCS Network plan (VitalCare Elite – HI 500)

Admittedly it’s a pretty sketchy plan, but I only need this short term and was the best option I found (live in NY). However, the plan’s documents are very vague and upon calling them, they said bloodwork would be “covered” so long as it was “routine” and I would get the “benefit of the PHCS Network”

I’ve done “routine” bloodwork before, only to get charged because the insurance company did not agree with the provider that it was routine, so I checked with this insurance’s claims team which said they could not tell me what would be covered 100% or not until they got the bill.

So now I’m in a bit of a pickle. I had previously been on Aetna and know my last bloodwork was charged at $4,000 to insurance (was on the hook for $200 in the end) and I will not be cool eating $4k for this test. Do you guys have any advice on where I can go from here?
