Botox Reimbursement Program + Insurance Company Not providing EOB

My insurance contracted pharmacy benefits will not cover Botox (the actual medication). I need the medication for the procedure.

Botox Reimbursement program requires:

  1. That I submit a receipt that I paid for the medication out of pocket after my insurance wouldn’t cover it (needs to be an explanation of benefits.

2.. That I submit the doctor’s billing of procedure to the insurance company.

Number 2 is easy. For number 1, my insurance contracted pharmacy company is refusing to send me an explanation of benefits. They said they can send me a “formulary listing” but refuse to send a explanation of benefits. They said they do not do this because Botox isn’t covered.. I’ve asked them repeatedly to send me something in writing saying they will not cover it and they refuse.

Help.
