Can someone explain to me the process used by a plan administrator to terminate United Healthcare COBRA coverage

According to United Healthcare my COBRA coverage was terminated by a plan administrator due to non paymemt. I have proof of payment for all COBRA premiums in the form of canceled checks my former employer deposited. The story I am being given is that it was a mistake made by the plan administrator who is a third party broker. Basically that they “hit the wrong button in the portal” could such an “accident” even be possible? If coverage was terminated would my former employer have been notified by UHC? Seems fishy and also seems logical that there would have to be a more deliberate process in place to terminate coverage than simply “we hit the wrong button and now you havent had insurance for the past 2.5 months despite paying your $1400 premiums”

Also just so everyone is aware I spent the majority of my day yesterday raising hell with the insurance company and broker and coverage has been reinstated.
