Dumb Question about Drug Copays

Shopping on the exchange right now and the healthcare.gov website explaining the plans available to me is listing drug copays. My (probably dumb) question: Are the copays monthly or yearly?

e.g., Suppose Plan A has 0 deductible and says that they have a $30 copay for a generic drug which normally costs $10 / month.

Does this mean that after three months of paying for it (reaching the $30 threshold), they start paying for it? Or does it mean that they never pay for that drug, since each month I spend less than the copay?
