Insurance charged differently for part of my routine physical?

I went in for my annual physical, normally covered by insurance.

During the physical, I asked the Dr about heartburn and we chatted for a few minutes before he wrote me a prescription.

A few weeks later a for a bill for a portion of the visit that the office charged differently. Apparently that discussion was not considered part of the physical and therefore insurance would not cover that.

I called the office and told it was an issue to discuss with insurance company. I called the insurance provider and they said to call the Dr.’s billing office…

Is this normal? Now I have to watch what I discuss or be charged for it? Really defeats the purpose of being able to have an open discussion about my health.
