Options For Someone In My Position

So I’ve recently accepted a remote position for a company whose based in a different state. Problem is, their offered Health Insurance has no in-network healthcare providers in my state (New York). The company does cover 100% of my monthly premiums, but the deductible for out of network is $3000/yr and I have to pay 50% for all of my healthcare needs from Primary to Urgent Care.

I do have a medical condition that requires me see a Primary Care doctor 4 times a year for routine check-ins/maintenance. So, my question is, what are my options? I’m pretty new to all of this, so any information would be greatly appreciated.
