out of pocket maximums, and bills. I need help making some sense of it all.

So my wife and I recently had our 2nd child. Bill has arrived, and it’s different than when we had our first.

First child the hospital contacted me prior to birth, let us know we had a $1,000 out of pocket max. Bill came and the amount was the 1k even. Straight forward and made sense

Fast forward a few years and kid #2 is born. Still have the same policy. Same out of pocket max. But the bill this time is more than 1k. How is that possible. Do I need to call insurance or the hospital or both?

I can look at my app for my insurance and it shows the out of pocket max is met.
