Told everything was covered at 100% and it was not

When I called (the provider not the insurance company) to get an estimate for a procedure (not preventative or anything like that) I was told that it was covered 100%. After the procedure I am now being billed for what my insurance didn’t cover. Who should I go to first? I have my itemized bill from the provider and the EOB from the insurer but how would I find out why they told me 100% and it is not? I don’t mind paying a balance – it just seems like a huge discrepancy to say 100% and then for it to not be. I did ask the provider and they said “it was only an estimate.” Not sure if this is worth pursuing or if I am just SOL.

I prefer not sharing the identifying information but I am located in NC and my insurance is through Aetna.
