What are the top rules to follow to prevent surprise bills and best make use of your insurance?

Hi! I’m a 28-year-old graduate student on the NYSHIP plan. We’ve had several instances of confusion with providers who say they take our insurance and are in-network, only to come back as out of network due to our specific plan (the empire plan). I have switched to using the provider finder often, but it’s a very clunky way to search doctors, and often one doctor will refer me to another who is out of network and the search begins again.

Given this, I wanted to ask (sort of starting from the ground up) how to use insurance. Like as if I know nothing – working from the very “teach me my ABCs of health care in America” level: What are the golden rules of using one’s insurance, making sure claims will be covered, and avoiding surprise bills?
