Denied state health insurance while unemployed! (PLEASE HELP)

Hi all,
I’m a single male living in Hawaii, I lost my Job back in March 2020 due to covid. I have a procedure that costs around $15k coming up and I currently have cobra extension through my work that costs $620/m. I currently can’t afford it because my unemployment ran out, I filed for extension and my payments are currently 5 weeks behind now.

I tried to apply for state health insurance called MED Quest, I got a letter from them that said:

“Not approved because income exceeds program eligibility limits. Your monthly household income exceeds the maximum income threshold of 138% of the Federal Poverty Level in Hawaii”

For some reason they counted my income as $2,807/m. I put down unemployment as income which is $2,592/m.

Does anyone have any ideas what I can do? Any Help will be much appreciated.
