Confused — how much am I liable for?

I’m about to start Invisalign and had the consultation including scans and signing paperwork on 15th Jan.

I was quoted $5,200 for the treatment by my orthodontist. $2,000 was expected to be covered by insurance (I had dentist call them, and so did I). I put $520 down with the remaining to be paid in 18 monthly instalments (just under $150 a month).

However, I just logged into the insurance provider’s website and saw an EOB that includes: – 3 line items for different types of imaging totalling $166. It says delta will pay for 2, I have to pay for the 3rd at $60 – 1 line item for comprehensive orthodontic treatment of the adult dentition. It says submitted fee $2,978. Accepted fee $2,056. Delta pays $1,028. I pay $1,028

So delta EOB in total is showing I owe $1,088. But the paperwork I signed with dentist said I would owe $3,200. Which one is correct?
