I received a claim denial that I checked was covered

I called my health insurance (AmerihealthNJ) in September for an October therapy appointment to see if it would be covered before I scheduled it. They said it would be covered so I scheduled five sessions. The first was covered and the next four were all denied. The second was covered and then later my insurance went back and denied it.

How do I fix this? The representative said I should have called to confirm they were covered and she saw that I had called and they were marked as covered initially. How do you know what’s covered and what isn’t? And if you call to check if it’s covered, they say yes, how do you know what’s actually accurate? If anyone could help me figure this out I would appreciate it.

I am 29.
