Hospital repeatedly requests copay that I’ve already made through my insurance provider?

For many, many months I get a few hundred dollar bill form the hospital for a copay that I already paid through my insurance provider (which is, I believe, how you are meant to do that. Not even sure the hospital should come after me directly for this).

Every month, I get my insurance and the hospital together on the phone

Every month, my insurance provider shouts at the hospital that the hospital has already cashed the payment

Every month, hospital makes some non-commital noise on the phone about going to review (but can’t give me a reference number for the call or review)

Every month, they will send me a fresh bill.

It is wasting hours of my life. How do I get the hospital billing department to fix their shit and/or switch to fighting with my insurance provider without me being in the loop every time.

Insofar as states matter, I am in Virginia and the hospital is in Washington DC.
