Wrongly charged by urgent care

So I got a Covid test last July (8 months ago) using my insurance (Blue Cross/Blue Shield) and paid zero dollars because, the tests are free with insurance.

I was aware the test would be free before I came and I even asked them, when they reassured me yes it would be free.

So this week, 8 months later, I receive a weird email that my card on file has been charged for $160 for that Covid test I took back in July.

First of all, how is this allowed? The test is free and everyone knows it. What exactly am I being charged for? Regardless, I spoke with my insurance about this and they told me that they already paid the urgent care for my test and they have written proof of it. BCBS doesn’t have a clue what I’m being charged for and when they try to reach out to the urgent care, they get the runaround and are sent to multiple different numbers, going in circles trying to reach someone who can actually give them answers until finally they hit the “billing department” who of course has a full voice mailbox and there doesn’t seem to be a human capable of answering the phone.

I’m at a loss here, I don’t get how this is legal. What sort of advice can anyone give me, should I March down to the urgent care in person and demand my money back (sarcasm, I know this won’t work)? Additionally, can I have them remove my card from file to ensure this can’t happen again?
