Insurance is charging me for Pap/HPV screening from my annual OB/GYN exam. Am I missing something?

Hi all. Perhaps I’m mistaken, but I thought preventive testing was required to be covered under all insurances now. I only recently reobtained insurance via my husband (we’re newlyweds), so it’s been a couple years since my last exam (like 5). We have gold level insurance through a prominent provider in our area. The appointment I had was nothing more than an annual and the doctor said the tests are part of that exam, yet the insurance is billing me. I spoke to the billing manager and she claims she submitted it correctly—twice, after I called her—and that the insurance is screwing it up. Insurance says it’s the office, but they’re being pretty vague about it so I’m suspecting they’re the culprit after all. What do I do, and am I misunderstanding something here? I’m 35 in Pittsburgh, PA.
