How do I go about getting a practitioner/facility to correctly document or amend the information from our encounters after my medical information from our interaction is harmfully incorrect, more than once?
This is important because incorrect documentation has participated in my health being mishandled and misrepresented to my detriment for over a decade.
I’ve never pushed for it to be corrected before but my rapidly declining health has forced my hand here. My faith in medicine and the patience I once had with it’s process has been long gone.

If this is helpful: It’s at a MTF in NV however I pay for Tricare and am not the military member so I can see practitioners elsewhere just not at a VA.
