Insurance company refusing to pay on valid claims

I’ve been seeing a therapist for a few months and have been using insurance to see her. I have confirmed multiple times that she is in network.

For the last few months, the insurance company has been really inconsistent about processing the claims, so that she doesn’t get paid. I say sporadically because sometimes they pay, sometimes they don’t. Sometimes their excuse for not paying is that she allegedly submitted the claim incorrectly and needs to resubmit it (This has happened as many as three times for some of the claims), sometimes they’ll a say she’s out of network, which simply isn’t true. I’m becoming more and more convinced that there isn’t actually a problem with the claims, and the issue is that they don’t want to pay

We’ve both called the insurance company many times to get this resolved, but it’s been going on for months. At this point she’s saying that she’s going to have to turn the bill over to me, which I understand. I fully expect reimbursement for them, my premium is around $250 a month, so I’d better be able to use my insurance. What’s the best way to do this? Should I report them to some agency? Threaten to take them to small claims court? My main goal is to get paid, but I’d also somehow like to show them that they can’t do this to people.
