Claim Completed but not Accepted by Provider??

I have been seeing a therapist now for a few months weekly and paying him a flat copay until my insurance went through. All of the claims are showing as completed on my side, and that I owe the copay (that I’ve been paying directly to the therapist. I assumed that once he received my claims, the copay issue would be resolved. However, he contacted me today to let me know he received a letter from billing that they don’t actually accept my insurance. I don’t understand how this is possible since the claim wasn’t rejected and each claim has been completed, but more concerning is that the insurance company now says I owe the total of 7 copays to them that were all already paid by me. I’ve contacted both the insurance company by email and my provider by text, but my provider is adamant that there is no issue. Continuing to pay him without insurance isn’t an issue as he has a sliding scale rate, but obviously I’m concerned about the current bill that my insurance company claims I owe. What should be my next steps, and can somebody explain this to me?
