What is the best way to get my Insurance company to properly cover my ER visit?

I live in the US. Over the last 3 months I have had to take 2 trips to the ER due to stress and exhaustion. I collapsed and had to be rushed to the hospital due to extremely low blood pressure.

Both incidents were terrifying. Both times I wasn’t coherent enough to get to choose which ambulance service or ER I was taken to. Both times I was admitted the max they kept me was 3 hours for observation (the second trip).

Now I’m staring at an ER bill where I can tell that my insurance has only covered 2/3 of the cost. My deductible for and ER visit is supposed to be $125.

I am going to call my provider this morning, but I want to come prepared.

How can I best go about fighting this without ending up in collections?

Further info, I have BCBS and I’m on a High PPO plan.
