Primary code “Behavioral health”

I’ve been back and forth with United and my provider and I’m TIRED.

This is my situation:

visit #1 in September 2020: went in for an eye infection + refill of anti-depressants *only paid co-pay

visit #2 in March 2021: went in for a pap test + refill of anti-depressants *pap is covered, but “mental health visit” isn’t (refill of my anti-depressants?) United is charging me for a full out of pocket visit.

I speak with the Practice Manger and she does not know why visit #2 is not covered. She explains they are both coded with the same “mental health” codes. She suggests maybe my coverage/plan changed?

I called United and CSR confirms that my plan with from visits did NOT change.

She explains that visit #2 wasn’t covered because the primary code was “Behavioral Health” and that the Visit #1 primary code was “Medical health.”

In order for United to cover it, the primary code needs to be “Medical Health.”

Can I get my provider to change the primary code to “Medical Health.” I feel like I shouldn’t have to pay for it all of a sudden. I could understand if my policy changed, but I was able to confirm with the CSR and further research, it has NOT change. Please help!
