Currently on COBRA, trying to make plans for what comes next

My husband (43) and I (42) are currently unemployed/self-employed/retired. We have 3 elementary-aged children, the oldest is a T1 diabetic. Because of where we live, the best care for him is across state lines. We are currently on COBRA and will be until sometime next year, paying through the nose for a super premium plan with United. When COBRA ends, I have no idea what comes next. We’re faaaar too young for Medicare, and we have the kids to consider anyway.

So here’s my question. Can I just… call up United next year and buy a plan from them? Like, do people do that? Because of my son’s diabetes, we very much want to keep his doctors and everything the same as long as possible, and all the plans in our state just don’t cover that facility because the plans are for this state. I tried talking to United a couple months ago and got a “talking head” call center guy who was completely unable to answer anything complicated. Plus, no one will talk theoretically with me about next year. They only want to talk about this year.

For the sake of argument, assume money is no object. I’ve poked at healthcare.gov. That’s not what I want.
