Date of service VS date of Claim received/processed

Hello, does anybody know if deductibles work by the “Date of service” or “date claim received/processed”?

Long story short i have a combined in & out of network deductibles, the first visit of the year was out of network and the amount should have covered the in network deductible however the claim was submitted late and took a month and half to be processed after received and in between i had like 5 claims in network processed and had to pay them.

I always knew that benefits generally applied by the date of service but i was told deductibles work with the date claim processed and they are refusing to re process these claims, i asked where can i see this in the plan book they said they don’t know it’s like that in all the US health plans.
