Help with understanding my HMO, PPO, HDHP (+HSA) options? They all look bad

I’m guessing #3 is the worst, while #1 & #2 look identical to me. The prices are all within $20-30 except #5, which is half the price of the other ones.

Should I look up the hospitals in the area before deciding? I’m in my 30s and don’t expect to have any medical issues but who knows..

How do I think about these plans? Why is #4 and #5 called high deductible when they all have the same deductible? Is a visit to the doctor going to be much more expensive with that plan? Is that why it doesn’t specify a copay? I pay 100%?
