Fertility test coverage

Hello all,

Me (28M) and my wife (31F) have been trying to have a baby for over a year now and since there is no sign of things working out this month I am starting to consider the possibility of doing more through fertility test. I’ve already done those sold in pharmacies and while my sperm count seems okay, I know that there are other variables that may decrease our chance of success, such as sperm activity, shape, etc.

My question is basically what is the chance of my health insurance (UHC, quite a premium coverage obtained through my university) covering the costs of meeting a doctor and being tested. The plan brochure states exactly this: “Infertility treatment (male or female), including any services or supplies rendered for the purpose or with the intent of inducing conception, except to diagnose or treat the underlying cause of the infertility”.

Since I don’t exactly know what is happening, this would be treated as diagnose, right? So it should be covered?

Also, sorry for asking another question, but since I am not from USA I am not so sure how things work here. Do I need to go to a GP first and then get a referral for a specialized doctor? Or can I just google a fertility doctor and schedule a meeting with him?

Thanks!!
