Impossible bill just went away?

So I got an eob after a routine procedure, but added onto this one was oral surgery that I supposedly had while undergoing a medically induced seizure. Literally impossible, right? I mean how on earth could someone perform oral surgery while their patient is seizing. I called Kaiser and they just said “well this is what they reported and coded so there’s nothing we can really do about it”. The facts that I didn’t have the surgery, and that it was impossible to have performed weren’t enough to prove it didn’t happen. I said I simply wouldn’t pay it. Received the bill, didn’t pay. Got another eob for the same visit in the mail, same oral surgery still on it, ignored the bill again, called and explained it wasn’t possible and the phone rep kinda understood like “yeah this doesn’t make sense” but there wasn’t anything they could do supposedly. Later got another bill for it and threatened with collections. Paid the bill, then a week later received a check back from them for the same amount I just paid. No explanation with the check. It’s been a few weeks now and I haven’t heard anything from anyone. Did this just go away somehow?
