After Hours Dr visit network question

We’ve just met our $14k Family OOP on our high deductible plan but did not realize it when visiting an after hours clinic. They said they were in network (website says it too), and said the cost would be $135 which sounds right toward deductible. Saw a physicians assistant and was happy with the results. Insurance claim was filed under the physicians assistants name and insurance says they are out of network, the facility also appears to be out of network according to the insurance search page – but their other nearby locations are in network. Insurances says we may owe around $250 for ‘New Doctor Visit, 45min’ rather than $0 if they were in network due to having met OOP.

I know it this happens with ERs, but can this happen at any doctors office with the primary physician you are seeing?
