Ankle Injury – Claim Question

Hi All,

I injured my ankle a few months ago, there was no specific accident or event, but it became increasingly sore and needed attention. It could have potentially been from running a few times a week. I ended up going to a podiatrist, then physical therapy, then an orthopedic. I just received a letter from my insurance company to further evaluate the claim filed by one of the providers I saw.

The closest options to the cause of injury are:

Wear and tear, developed over time Recreational or sports injury (i.e. bicycle, soccer, play equipment) Spontaneous onset (no accident or injury, cause unknown).

There is also a space to provide more information, regardless of which selection I make.

Does anyone have any advice on what information would be best to include in order to ensure this claim is not denied? It is a legitimate injury that has been painful for a few months and I’ve had multiple types of treatment. If this gets denied I would be on the hook for thousands of dollars, so I just want to make sure I don’t screw up on this form.
