Hi I have to pick a plan on the form. I picked CA Health and Wellness.
Then there is a line where I have to put a doctor/clinic code. What if I don’t have a doctor or clinic picked out yet? Or what if I want to visit multiple ones for different things? Like one for behavioral health, and another for dental?
Can I just leave it blank?
