My insurance (Blue Shield of CA) coverd at home covid tests if they are ordered by a healthcare provider.
Since appointments for Covid testing are impossible to get at my doctor’s office right now (and at many testing sites due to the surge), I was able to get my doctor to write me an order for one of those at home covid tests.
However, the claim forms for insurance reimbursement have required fields (like provider’s tax number and procedure code) which my doctor isn’t willing to provide for reimbursement purposes because they technically aren’t the provider — CVS where I am picking up the over the counter testing.
Called my insurance company and they said to put my doctor’s information but I don’t really trust the customer service reps because each one tells me something different.
Anyone successfully claimed reimbursement for at home covid testing? How did you fill out the claim forms?
