What are the most common ways people in USA end up paying out of pocket for something that they thought would be covered by insurance?

Ex: what about their insurance or situation made them think insurance would cover it, but actually it didn’t?

Recently I went on a journey to figure out how to get some lab tests done in a state outside of my insurance area. I think to prevent paying out of pocket next time, I have to see my primary care doctor for a visit while I’m in her state and then ask them to write a lab order to do in a different state if I need to buy out of my home state for some time? Not sure
