On my health insurance enrollment form from my job it asks for all medical diagnoses and treatments from past 5 years. If I saw a therapist weekly but he didn’t “believe in diagnosis”, do I leave that off and is that not “misrepresenting or leaving out information”? I don’t want my insurance to get terminated but I also have no idea what I would write!
