Hey all, my elaboration continues directly from above. If say John Doe was discovered to have several tumors in his leg and proceeded to get them removed/shrunk etc-> (who) and how within the insurer is the decision made to accept or reject coverage charges for the procedure? As in, what is the title or management level of the person in charge or is the basis of my question entirely off? Is it automated algorithmically based on that person’s information + other cases?
