College Student with 1.5k bill. Now what?

So I recently went to the cardiologist, where he did a stress test with 2 EKG. I am enrolled under my dad’s insurance plan, but it turns out the deductible is 2k and not the no-deductible plan. I now have a bill for 1.5k that I can’t pay. The hospital told me to fill out the financial assistance program application and include a letter that states that I am a college student with no income. Should I include in this letter the fact that my dad pays me 1.5k a month for rent/food? Should I just leave it at “I have no income?”

Does the money my dad pay me count as “wages?”
