Will my insurance cover an old knee injury?

Around 4 years ago I partially torn my ACL, my MCL and meniscus. I was recommended to do Physical Therapy and it went well.

Right now I’m having instability problems and sprains even if my knee and leg is strong so I may need some kind of treatment or surgery for this.

My current insurance is through my employer and I have been with it for over 1 year.

Is this a preexisting condition? Will my insurance cover this even if is a preexisting condition?

I would like to go to the doctor but I don’t know if should I explain the whole situation or this will make my insurance to not cover it
