A claim the hospital sent my insurance was denied by my insurance because (456 – MEDICAL NECESSITY FOR CARE NOT MET. PROVIDER LIABLE) On my EOB it says my responsibility is 0.00. Does this mean this hospital is not going to be paid? I’m concerned this will come back to me, it’s $76,000.00.
