How exactly do the terms ‘amount approved’ and ‘amount paid’ work on a health insurance receipt?

So my initial guess (which I’m guessing is wrong) is that amount approved is the negotiated amount between provider and insurance, and amount paid is what the insurance pays the provider, which makes sense to be the same amount as approved

But for some reason my amount paid is always significantly less than the amount approved. Does anyone know why this is? Am I understanding how each of these terms work incorrectly? Member responsibility is always $0, so it’s not like the difference is forwarded to me
