Do I contact the provider or my insurance in attempt to get refunded?

Hello everyone. I’m not are what all information would be needed, but my Fiance and I had a baby recently and her employers insurance did not cover anything in regards to pregnancy. Due to this I was paying out of pocket for our visits totalling about 3200 dollars. One of which was a single 2500 dollar payment for what we were told covered delivery and post delivery expenses. Coming into the middle of the pregnancy my fiance left her job to focus on her and the babies well being and then got into Tenncare/Bluecare through the state of Tennessee. After this we paid nothing out of pocket. However after looking at the itemized billing it seems Tenncare was charged 4,931 dollars for “Routine Obstetric Care” at the date of our child’s delivery. Seeing as our charges added up to be 4,938 dollars before the self-pay adjustments, I’m thinking Tenncare paid for all of our appointments and delivery in one lump sum? Even after we paid out of pocket, which would entitle one of the two paying parties a refund? I’m not really sure, but I don’t want to leave 3200 dollars on the table if so and was hoping someone more knowledgeable could point me in the right direction. Does it sound like a refund might be in order? Do I need to contact the provider or the insurance in hopes of achieving that? Thank you in advance and sorry for the lengthy read. 🙁

TLDR: I think we paid 3,200 for our pregnancy, followed by our insurance paying 4,900 dollars for the same things. I’m hoping to find out if this is correct, if there’s grounds for a refund, and who to talk too to make it possible.
