can my covered california coverage be terminated without notice?

I’ve been on cov ca for three years with out issue. In October, I renewed my plan as usual but indicated some changes in income and uploaded proof of income documents. At the time, it appeared as though my renewal application was accepted and I received notification of my new premium for 2022. I thought I was all set. Then, yesterday, on December 16th I received an email notification that my health insurance plan through cov ca will be terminated effective 12/31/21 (per my request according to letter). I did not request this! What is going on? Can my coverage be terminated with just two weeks notice without explanation? No one at cov ca can help me as they say “the county” terminated my plan but I have not received any notification about why, what my eligibility is and what do I do next? All I know is that I have no health coverage after the end of the year (less than two weeks!).
Does anyone have any insight and/or advice as to how I get this resolved? Thanks for any and all input!
