Picking a healthcare.gov plan, expecting surgery, quality of provider

I need a hip surgery and maybe some kind of heart procedure, a pace maker maybe. I don’t think it is panic attacks.

I am really ignorant with all this stuff.

So i think i have to get a plan from healthcare.gov. No income, not employed.

If I choose the Mayo Clinic as a provider, the plans are more expensive, like +$1000 per month.

Does that mean I would be able to choose and decide to get the surgery at the Mayo Clinic if I want to. or any other major procedures? Like I get to make the decision?

The cheaper plans are like $13,000 total yearly expense versus $18,000 for a Mayo clinic plan. I don’t know if it is worth it.

Anything else I should think about?

Thanks
