In November I had to purchase shitty short term health insurance to cover a gap between jobs. Literally a 2 week gap, but it took another month for new benefits to kick in. During that time under the short term plan through Golden Rule, I had to visit the ER for something completely out of the blue. I was sent paperwork by the insurance company to fill out to file a claim. After I submitted that, the insurance company is requesting medical records from my primary care provider and the ER doctor (see images below)
I’ve never dealt with this before. Do I call up my PCP office and ask them to send records? Same with the hospital ER records? Where do I tell them to send it? Just looking for general advise to explain what the letters mean and what I need to do next. Thank you so much.
