Confused about CCR forms with Kaiser

Hello all, I am 18 years old and on my parents insurance (Kaiser Permanente). I live in Northern California with my parents.

I want to be able to use my insurance to access services (through planned parenthood) without my parent’s knowledge, so I want to make sure that I have everything confidential before I use these services. Access to these services is not time sensitive, so I’m mainly concerned with having everything 100% private.

I already changed everything I could through the Kaiser app to be paperless and go to my email, so that I don’t have to worry about them opening my mail. This included my explanation of benefits.

Here are my questions:

Is this sufficient to ensure my privacy, or do I also need to fill out a confidential communications request with Kaiser? Is there a CCR form specific to Kaiser that I need to fill out, or can I just use a generic one I find on the internet? (link) I found one that is specifically for Kaiser (link), but appears to be for SoCal. Can I fill this one out? If I do, do I send it to the address at the bottom of the form (in SoCal), or the address listed on their notice of privacy practices document.

I’m having a really hard time understanding all of this, so any help would be appreciated.
