United Healthcare not giving me reimbursement I’m owed

My United Healthcare plan covers chiropractic visits up to a certain amount (I believe $200). All they need is an itemized bill with some codes and they should process it. Well, I sent them the bill back in September and they have yet to reimburse me. Every time I call, they give me some BS reason why they haven’t processed my claim. Last time I called, the woman said benefits department did receive the bill but the claims department has yet to process it. I’ve sent a copy of the bill to three different benefits members, and I’m about tired of them coming up with some BS reason why they haven’t processed my request.
