Primary doctor keeps billing mental health code and my health insurance is denying it, don’t know what to do

Ever since I started seeing him, I would have to call the hospital physician billing department to get it corrected. Usually one call and it would be fixed. My EOB always states the same thing that it’s denied and needs to be filed under the mental health side of my insurance.

He’s in network for my health coverage which is a Cigna provider. My insurance has a separate side for mental health (Optum Health, but not really relevant here since they shouldn’t even be submitting a claim to Optum). My internal medicine doctor does not do anything for mental health. He’s in network under my health insurance. I see him every 3 months as I have a chronic illness.

This time I have called the hospital billing department 3 times, spoke with the office manager once, and just left another voicemail. My last appointment was early NOV, and the bill is now overdue. I met my out of pocket max last year, so I shouldn’t even owe anything for this visit.
