Why would a generic drug be considered Tier 4 Non-preferred on all part D Medicare plans I’ve looked at?

Specifically: Sucralfate oral suspension. The liquid version has been generic since 2019.

There is a pill version that is so huge it’s impossible for my husband to swallow after his esophagectomy.
