I personally believe they should only be sent upon request. Every time I receive an EOB (estimate of benefits) I want to have a goddamn heart attack – because every “estimate” is PAINFUL to look at (unless it’s just me and my shitty insurance… but from things I have read on this subreddit I am optimistic that I am not alone). I am real tender about this right now because I just received an EOB that says what I “might” owe is several hundreds dollars more than the estimate I received in the doctors office. I believe if you have to use the word “might” in a document – you “probably” shouldn’t send it.
