Do health insurance companies give doctors a hard time if they prescribe hormone therapy for women in peri menopause or menopause?

Despite new evidence that shows HRT is actually beneficial for women going through hormone changes doctors act as if you are asking for pain meds and push back constantly. After hearing about insurance regarding late Lyme disease and how people have to go to doctors that don’t take insurance, is this the same? Insurance companies push back or hits the doctors in a negative way? I can’t think of any other reason why the fight when you feel awful for a couple years, exhaust every other option and have to fight for them. What gives? I am starting to think this is a money/insurance thing for doctors.
