Hi everyone, husband and I are trying to file a critical illness claim because I was diagnosed with endometrial cancer last year. Currently in treatments. Our insurance company (Prudential) asked for a list of providers from 10/13/2020 – 10/13/2021. I’m not really sure how to provide this. I mean do I just type out my providers name and contact information on a Word doc and mail it in? Or do they need a more official paper? If so, any ideas on how to get it? I just mailed in everything else they asked and also requested my medical records to be released so this is the last thing I have to do. Any advice is appreciated! Thank you!
