No primary care doctors in my area, who can I talk to about this?

I have a marketplace insurance (Bright HealthCare) and I’ve been trying for over a month to contact them regarding establishing a primary care physician but I haven’t heard anything back! Today they called me trying to get me to sign up for their rewards program (side note: health insurance having a rewards program?? wtf) and one of the rewards requires me to establish a primary care doctor.

Well, every single person I have talked to, including the person today, and Bright’s online portal, say the exact same thing: There are no primary care physicians within 100 miles of me. It’s infuriating because every single time I bring it up, they search for one themselves and every physician that shows up is either a gynecologist (specialist) or pediatric care (i am 21). I’m lost on what to do or who I can talk to. I’ve called their customer care multiple times and each representative I have talked to claims they write in a “note” on my file but nothing ever happens? What can I do?
