Questions About Weird Bill

My college-aged daughter was seen last summer in the ER for nausea and vomiting. It turned out it was a UTI. She was seen in June. We got the ER bill in July and paid our portion. Today I received a bill from Envision Physician Services for over 1k. The bill states they do not have an insurance on file so they are charging us as self-pay.

She is on our Tricare. I have the Tricare summary explanation of benefits and it shows that the physician services already charged Tricare, and Tricare paid (this was in addition to what the ER dept of the hospital billed). There were two charges on the summary. On the bill there are three charges. The numbers on the bill, and those shown on the summary do not match at all. The summary shows the amount we should owe provider is 0.00. Also, the summary shows the physician services was billed by a different entity than what is being shown on the bill. I’m very confused.

Also, they completely upcoded. They have the ER eval & mgmt as a level 5. My understanding is a level 5 is pretty dire. She was simply really sick. We waited in the ER for six hours before she was seen. Clearly, she was not near death (even though she may have felt like it). Also, they are charging us $69 for putting a pulse oximeter on her finger!! Can they charge for that?

I’m calling Tricare in the morning, but thought maybe someone here has insight. Should I call the place that is billing me first and deal with them, or go directly to insurance? I wasn’t sure they could help since it doesn’t look like this place filed with them in the first place.
