What kind can of evidence can prove that I didn’t have a pre-existing condition for an outpatient surgery I had? The insurance company approved the surgery, but are now denying all claims. The surgeon records don’t say that I had this problem (which I didn’t), but when I went to the ER I couldn’t understand all the questions they asked me and they wouldn’t allow my wife to come in there with me.
